PCF Funded Converse County Voucher program

Program Summary:

Develop a process to approve and distribute the WBCI Early Detection Vouchers using the following criteria:
The USPSTF breast cancer screening guideline recommendations.
Individuals who are ineligible for the Wyoming Breast and Cervical Cancer Program.
Individuals who live in Wyoming.
Individuals who declare a financial hardship and an inability to pay for a screening mammogram.
Individuals will not be denied based on their gender, age, income, or insurance status.
Provide referrals to the Wyoming Cancer Resource Services for patient navigation when a screening mammogram identifies a need for follow-up.
Send patient reminders using electronic health record data.
Establish and implement a marketing plan to increase awareness of the program.
Complete data tracking and reporting on services provided.
Attend quarterly meetings with WBCI.
Provide reports on services rendered.

WBCI & DOH Voucher Partnership

Program Summary:

WBCI would like to award the Wyoming Health Department, Wyoming Cancer Program grant funds for breast cancer early detection screening mammograms, diagnostic mammograms, and ultrasounds screenings and diagnostic testing. Details of this partnership are outlined below.
Award Period.
The start of the award period will coincide with the earliest date possible after the completion of the necessary paperwork. Expected start date would be in the middle or latter part of May 2021.
The end date for this award will be December 31, 2022.
Funds Awarded.
Starting funds of $10,000 will be granted to begin the program.
Additional allotments of $5,000 increments will be available upon request and receipt of financial records documenting disbursement of previous funds.
Funding for the award period noted above will not exceed $40,000.
Criteria For Spending the Funds. Funds will be allocated:
To individuals living in Wyoming.
To individuals meeting the USPSTF breast cancer screening guideline recommendations OR to patients who have an underlying condition that the patient and their medical provider believes necessitates an early detection screening mammogram, diagnostic mammogram or ultrasound.
Without any limitations based on the recipient’s gender, age, income or insurance status.
To individuals stating they have a financial hardship and are unable to pay for a screening mammogram, diagnostic mammogram and/or ultrasound.
